CERVICAL LENGTH IN PREGNANCY  

There is a wide variation in the length of the cervical canal when measured transabdominally with a full bladder. Overdistention of the bladder gives one the impression that the cervix is elongated due to compression of the anterior and posterior uterine wall. Similarly an empty bladder results in the cervix being more vertically orientated and falsely short.

Transvaginal scanning avoids the problem cervical distortion and is probably the most accurate method.

The normal mean cervical length is >3 cm until 32-34 weeks of gestation. Thereafter gradual shortening and effacement occurs. There is no significant difference in the cervical length of primigravid and multigravid women (1,2). Progressive shortening of the cervix is more significant than a single abnormal cervical length measurement (3).
